With it in 2 gears ( put it in 1st then 3rd with the sleeves)
a boxed end wrench tight firmly by arm , (both parts are hardend it stops when it bottoms out) with loctite usually , then a center punch chissel to set the lock on the nut to the shaft channel.
We had 5th gear nuts still loosen on the threads ,sometimes 2-3 k after rebuild

I think the issue here may be there is no lock nut, some early boxes didn't have them.

Locking it in 2 gears is correct, but you need a way to torque it properly.

Wailing on rare/expensive trans parts with a hammer and chisel will put you in the Sins of the PO hall of fame..

if you clean the threads spotless/dry and use red permanent loctite at factory torque, it will not fail, but everything must be clean&oil free.

There are other more elegant ways to lock it in place, but they are all more involved. Either way, you need an
accurate way to tighten it or it will fail after a few thousand miles.

BTW the torque spec is 122-145 ft-lb. Use 125 with loctite and the effective torque value would be over 150. Unlikely that will ever come loose if prepped properly. If you go crazy on the torque (big impact etc.) you may damage the main shaft. Would not want to do that.

Here is one type of wrench:

Hey, also i noticed on your photo, (the single nut photo), that the factory lock tab is missing. If it was run without the lock tab and no loctite before, that is almost certainly why it failed- that is pretty much certain failure.
